Immunization Requirements for Incoming Students

This is a required document.

Influenza

One of the following is required:

• Documentation of an influenza vaccination for the upcoming flu season due no later than October 31 OR
• Declination waiver is acceptable for valid medical reasons only.

The renewal date will be set for Oct. 31 of the following flu season.

Varicella (Chicken Pox)

Submit a positive, quantitative antibody titer (lab report required) for varicella.

If your titer is negative or equivocal, new alerts will be created for you to receive one booster and provide a second titer.

Measles, Mumps & Rubella (MMR)

Submit a positive, quantitative antibody titer for all three components (lab reports required)

If any titer is negative or equivocal, new alerts will be created for you to receive one booster and provide a second titer.

Tetanus, Diphtheria & Pertussis (Tdap)

Submit documentation of a Tdap booster within the past 5 years.

The renewal date will be set for 10 years from the administered date of the booster.

Hepatitis B

Both of the following are required: 

• Three vaccinations AND
• A positive quantitative antibody titer (lab report required) or a positive quantitative antibody titer (no lab report) and a negative surface antigen.

If your series is in process, submit where you are in the series and new alerts will be created for you to complete the series and titer.

If your titer is negative or equivocal, new alerts will be created for you to repeat series and provide a second titer.

Tuberculosis (TB)

One of the following is required:

• A negative two-step PPD test series (administered one-three weeks apart) OR negative IGRA (either must be within six months of arrival at BSOM)
• If positive results, provide evidence of a clear chest x-ray

The renewal date will be set for one year. Upon renewal, one of the following is required annually:

• One step PPD test OR negative IGRA
• If PREVIOUS positive results, provide completed TB Questionnaire
