Read our frequently asked questions, in which we answer many common questions about the Anatomical Gift Program.
Complete the Vital Statistics Form (PDF) as accurately as possible. Upon your death, information from this sheet will be used to initiate a death certificate. Please pay particular attention to the accuracy of dates, spelling of names, and your social security number. All blanks on the Vital Statistics form MUST be completed. In addition, it is the donor’s responsibility to contact the Anatomical Gift Program office with any changes of information.
Complete the Anatomical Gift Consent and Release of Ashes Form (PDF), Parts 1 and 2.

Once your information has been entered into our database, we will send you a donor card including your name and donor number as well as the phone number to be called at the time of your death.
