Departmental Guidelines

Chairs and their designees are responsible for the duties listed below. See the Activity Timeline for submission dates.

Department Chair

  • Review CME applications and endorse memos of agreement
  • Appoint a department CME coordinator/liaison for approved activities
  • Provide direction or delegate responsibilities as needed for approved activities

Department Coordinator/Liaison

  • Complete and submit the CME application in conjunction with the activity chair
  • Obtain appropriate agreements from exhibitors and contributors | Exhibitor Agreement (DOC), Contributor Disclosure (DOC)
  • Obtain disclosure and attestation documents from speaker(s) | Conflict of Interest Disclosure (DOC)
  • Review and obtain endorsement of application by Chair
  • Make travel arrangements for out-of-town speakers
  • Arrange for audio visual needs and instructional materials
  • Reserve location
  • Record and verify physician attendance at CME activities. Records should include:
    • a legible listing of individual attendee names
    • the total number of physicians and non-physicians
    • any deviation in the length of attendance by an individual
  • Submit summary of evaluations
  • Monitor expenses and prepare financial statements
  • Prepare certificate for attendees
  • Maintain activity file for six years
