PACER Toolbox Kit

Professionals Accelerating Clinical and Educational Redesign (PACER) was a professional development program designed to help an interprofessional team of primary care clinicians, educators and staff obtain skills needed to transform a traditional primary care training environment into a patient-centered medical home. The project aimed to assess the impact of an interdisciplinary interprofessional faculty development program on: 1) individual participant skills including patient-centered care, leadership, quality improvement, interprofessional care and education, stewardship of resources, and competency assessment; 2) transformation of health professional training and primary care practice, and 3) organizational changes toward building sustainability. PACER is a national program, administered and evaluated by a team of experienced educational researchers based at Oregon Health & Science University (OHSU).
